You may download our application and submit it by fax (941.625.5167), scan and email (info@LivingWatersPreschool.com) or drop it off at the church office Monday-Friday 9 a.m. to noon along with the registration fee, copy of birth certificate, immunication record (blue form from doctor) and medical exam form (gold form from doctor). VPK parents also turn in VPK voucher from the Early Learning Coalition.
